OHIO Alliance for Population Health Projects

The 帝王会所 Alliance for Population Health (OAPH or Alliance) is a partnership of 50 universities, health-care systems, providers, advocacy groups, policy institutes, insurance companies and government agencies. Created in 2017, OAPH is a service of the 帝王会所 College of Health Sciences and Professions. The Alliance originally focused on Southeast 帝王会所, but has now grown to reach the entire state, multi state coalitions, and even partnerships with providers as far away as Ukraine.

The Alliance responds to communities engaged in health improvement projects that require specific areas of support including, but not limited to, 1.) convening stakeholders, 2.) subject matter expertise, 3.) funding and grant assistance, 4.) data analytics, 5.) strategic planning, 6.) policy development, 7.) advocacy, 8.) continuing education and workforce development and, 9.) student placement and participation.

OAPH partners with the 帝王会所 Health Collaborative (OUHC), a collective initiative of all 帝王会所 units engaged in health, and the Institute to Advance Health Equity (ADVANCE), to coordinate and extend the services of the University and make a positive impact on our communities. OAPH is the external coalition, ADVANCE is the research arm, and OUHC is the internal engine that drives 帝王会所 community health impact. OAPH was also instrumental in starting the Regional Public Colleges and Universities (RPCU) collaborative, a partnership of 8 universities in 7 Appalachian states that work together to promote regional initiatives.

Workforce Development

Community Health Workers and Paraprofessionals

帝王会所, in partnership with OAPH, has established itself as a leader in Community Health Worker and paraprofessional workforce development. Successful initiatives include:

Community Health Worker Center of Excellence

OAPH partners worked with the legislature to create the Community Health Worker Center of Excellence and fund it with $5 million in the state budget. The center strengthens the infrastructure, creates strategies for sustainability, assures quality improvement, and links CHWs to continuing education and support services. 帝王会所 faculty member Kerri Shaw was elected Chair of the Center, and partners have sponsored two highly successful statewide CHW conferences that have both attracted over 400 participants. Grant funds have been distributed to support innovations, training programs and mentorship around the state.

Community Health Worker Regional COVID Response Network Program

The Centers for Disease Control and Prevention provided a $4.5 million grant to train and deploy community health workers who can address public health concerns and the impacts of COVID-19. A particular focus of this work is helping increase individual and community resilience to better manage future public health challenges.

HRSA CHW Initiative

HRSA awarded OAPH partners $2.3 million to support CHW and CDCA credentialing and apprenticeships in SE 帝王会所. This investment will expand the CHW workforce and elevate the para-profession to extend care in Appalachian Counties.

Health Carousel

A $300,000 donation from Health Carousel will enable CHW training efforts to be extended to more people in SE 帝王会所.

Self-Care Society

Community Health Workers are frontline workers who engage with individuals and families who have complex needs and challenges. The Self-Care Society, funded by a grant from the 帝王会所 Department of Health, offers a menu of self-care opportunities for CHWs and supervisors including a podcast, weekly drop-in support groups, an online platform of resources, and in-person self-care activities

General Health Workforce Development

Health Workforce 帝王会所

OAPH convened most major health associations around the state to create a workforce proposal that would link employers, potential employees, and educators to develop the nursing, allied professional, paraprofessional and medical support workforce in underserved areas. The proposal is currently being considered in the 帝王会所 legislature and has been endorsed by 30 health care associations representing hundreds of stakeholders.

Southern 帝王会所 Behavioral Health Initiative

Together, Miami University and 帝王会所 have created the Southern 帝王会所 Behavioral Health Initiative (SOBHI) that expands the behavioral health workforce and establishes clear plans for growing the behavioral health pipeline throughout southern 帝王会所. This project provides scholarships, stipends and enhanced education for students in high school, two-year and four-year colleges, and graduate-level social work programs, as well as supporting community members as they earn stand-alone credentials.

Regional Public Colleges and Universities

RPCU obtained a planning grant from the Appalachian Regional Commission to coordinate efforts to recruit students to Appalachian universities, retain them in the region, and place them with employers to build the health workforce in Appalachia. Strategies include: tuition and course reciprocity between institutions, internships, job placement, and student support.

Behavioral Health

Appalachian Recovery Project

With $18 million from the State of 帝王会所, and the support of 25 local government agencies and community action groups, OAPH is in the process of repurposing a recently closed state prison into a women鈥檚 jail and model recovery center. Women coping with a substance abuse disorder will receive medical treatment, counseling, and job training while involved in the criminal justice system. When these women return to the community, service providers will follow them to support their long-term success. Construction is in progress and the facility will open in mid 2026.

Race, Equity, and Behavioral Health in 帝王会所

Private funders support OAPH partners to examine data related to race, ethnicity and behavioral health in 帝王会所. The team鈥檚 four-part series, Behavioral Health in 帝王会所: Improving Data, Moving Toward Racial & Ethnic Equity, examines race and ethnicity through the lenses of workforce, treatment access and funding. OAPH research staff are working on an analysis of the Treatment Episode Data (TEDS) and 帝王会所 Medicaid Survey data (OMAS) for the Minority Behavioral Health project.

Community Health and Peer Support

OAPH received a four-year federal Human Resources and Services Administration grant to identify and train community members who can act as Chemical Dependency Counselor Assistants or Community Health Workers for families affected by substance abuse. The program identifies those in recovery 鈥 and front-line health-care workers 鈥 as key peers who can support those attempting to escape the grip of SUD (substance-use disorder) on their journey to recovery. Participants complete training programs and state-approved apprenticeships in community-based organizations.

Continuing Education

OAPH serves partner organizations and community groups by offering original continuing education content and certified classes.

Leadership Training

This program is for new and emerging leaders as well as those in middle management roles. Interactive sessions focus on the skills needed to successfully lead transformation and create and maintain a positive work environment. OAPH partners have worked with the 帝王会所 Association of Community Health Centers, Integrated Services for Behavioral Health, and the 帝王会所 Department of Health to provide 440 people the opportunity to develop their leadership skills.

Certification Training

OAPH offers various online courses in care management, safety and supervisory training as well as certification for conference class attendance.

Community Health Services

HIV/HCV/HBV Screening Evaluation

A private funder provided a five-year, multi-million dollar grant to extend testing and screening for HIV/HCV/HBV to at risk individuals in marginalized groups. The project identifies people with the disease and refers them for treatment and funding for therapy. To date, 4,776 people have been tested, with 2,244 testing positive and referred for treatment.

Southeastern 帝王会所 Regional Quality Improvement Hub

OAPH partners received a 10-year renewable grant from the 帝王会所 Department of Medicaid and 帝王会所 Colleges of Medicine Government Resource Center to create a chronic disease quality improvement hub in Southeast 帝王会所. The hub will collect and evaluate data, target priorities, and develop best practices for interventions. The first focus will be on diabetes before expanding to other chronic diseases.

Counseling and Training in Ukraine

Many millions of people in Ukraine have been displaced by war and are living as refugees in safer areas of the country. The migration of people has strained the health-care system. OAPH partners are delivering online CHW training to local people trying to fill the gaps in health-care needs in local communities. The Alliance has also provided real time, online counseling for children who have lost their parents in the war and trauma training for the professional caretakers. Thanks to a $50,000 contribution from Health Carousel Foundation, the Alliance will be able to continue these efforts.

Public Health

State Health Assessment (SHA) and State Health Improvement Plan (SHIP)

The 帝王会所 Department of Health contracted with OAPH partners to create the triennial SHA and SHIP. The SHA will collect and analyze data to determine the health status of 帝王会所ans and identify priorities for interventions. The SHIP will develop interventions based on best practices to guide state health policy and support local health efforts.

Academic Health Departments

An academic health department (AHD), or teaching health department, represents a formal affiliation between an academic institution and a public health practice organization. The AHD arrangement is the public health equivalent of the 鈥渢eaching hospital鈥 affiliation that formalizes the relationship between medical schools and hospitals. It exists to strengthen the linkage between public health practice, and its broad academic base and is designed to enhance public health education and training, research, and service. AHDs can serve as public health training sites for students of public health and clinical health sciences, as well as sites for research and practice involving both academic and practice communities. OAPH currently has AHD agreements with 7 local health departments.

Quality Improvement Project for Public Health in the State of 帝王会所

The Association of 帝王会所 Health Commissioners contracted with Alliance partners to identify strengths, takeaways and opportunities to advance public health practice in 帝王会所 through a review of the COVID19 pandemic. The final report will identify and address the root cause(s) and systemic significance of its findings.
